How Out of Network Coverage works:
We collect $150 per visit in office at time of service until the out of network out of pocket maximum is satisfied. We then will submit the claims to your insurance company for you.  It is your responsibility to check if your insurance policy has out of network benefits, requires authorization, requires a script from a physician, or has visit limits in order to avoid denial of claims.  This can be easily checked in advance of your appointment by calling the customer service number on the back of your card.  We strongly recommend getting the agent's name and call reference number for your records.
